One Worker Dead, One Missing in Fairview Sewer Fume Accident

The Occupational Safety and Health Administration (OSHA) is investigating an accident in which two workers disappeared into a sewer line to unplug a pipe and did not emerge safely. At this time, one man has been found dead while the other man remains missing.

According to CBS News, the workers were completing a year-long project for the North Texas Municipal Water District and were contracted from the S.J. Louis Construction of Texas, Ltd. They were working on a location just north of Dallas, replacing a plug on a sewer line on their last day of work. One man went in without first testing the air quality or using a mask and did not reappear. A second man then went in after him, also without testing the air or breathing equipment. When he did not return, the other workers realized an on-the-job accident had likely occurred.

An initial investigation suggests that the workers may not have taken their usual safety precautions because they were finishing up a job. In usual circumstances, it is standard procedure to take safety measures related to the underground air quality and oxygen levels.
